About the Role
Wheatley Care is committed to delivering high-quality, person-centred support that enables people to live fulfilling and independent lives. We are currently looking for a Health and Wellbeing Assistant to join our Personalised Self-Directed Support Service – Mental Health.
The service is registered with the Care Inspectorate and provides housing support and care at home services to adults and older people with mental health needs, substance use issues, physical disabilities, and other vulnerabilities, supporting them to live independently in their own homes.
As a Health and Wellbeing Assistant, you will provide practical and emotional support to individuals, helping them achieve their personal outcomes and maintain their independence. Working in line with the Health and Social Care Standards, you will adopt a trauma-informed, strengths-based and non-discriminatory approach, supporting people to build confidence, resilience and connections within their communities.
What You'll Do
- Work in line with Health and Social Care Standards and organisational values;
- Build positive relationships with the people we support and encourage independence, choice and control;
- Deliver person-centred support tailored to individual needs and outcomes;
- Support individuals with daily living activities, including maintaining tenancy responsibilities, budgeting and accessing services;
- Assist individuals to engage with their communities, develop social networks and participate in meaningful activities;
- Promote wellbeing, healthy lifestyles and positive mental health;
- Administer medication where appropriate and in line with organisational policies and procedures;
- Record and maintain accurate information, support notes and documentation;
- Work collaboratively with colleagues, families, carers and external agencies; and
- Contribute to a safe, supportive and inclusive environment.
What We're Looking For
Experience & Skills
- Experience supporting vulnerable individuals, including people with mental health needs, addictions, homelessness, learning disabilities or other support needs;
- Understanding of person-centred support and promoting independence;
- Good communication and interpersonal skills;
- Ability to build positive relationships and maintain professional boundaries;
- Ability to work independently and as part of a team;
- Good organisational and time management skills; and
- Basic IT skills, including Microsoft Outlook, Word and Excel.
Qualifications
- Willingness to work towards a relevant SSSC-recognised qualification if not already held;
- SVQ 2 Health & Social Care (or equivalent) desirable; and
- SSSC registration required upon commencing in the role.
