We help older people live the life they want. We believe everyone has the right to live well at home in a way which meets their needs and aspirations, and those of their families.
What we do
We offer flexible, tailored care and support to older people, where and when they want and need it, suited to their individual requirements.
Our service is proactive, and focuses on helping older people lead healthier and more active lives, staying in their own homes and communities for longer.
On a practical basis, this means we can provide low level support to enable older people to stay as active as possible and feel part of their community.
We also provide home from hospital support, enabling older people to leave hospital as soon as possible, and helping them to settle back in to their homes.
