We currently have an exciting opportunity for a compassionate, motivated and person-centred Health and Wellbeing Coordinator to join our Edinburgh Housing Support Team.

The Edinburgh Housing Support Team is a registered outreach service providing temporary, fully furnished accommodation to people experiencing homelessness across Edinburgh. Working in partnership with Wheatley Homes East and The City of Edinburgh Council, we provide accommodation alongside regular, person-centred support, helping individuals achieve greater stability and independence throughout their accommodation journey. The service operates Monday to Friday, 9am to 5pm, ensuring consistent support is available to the people we work for.

As a Health and Wellbeing Co-ordinator, you will be the named keyworker for individuals accessing support and will utilise the Outcome Star assessment tool to co-create personalised support plans to encourage people we work for to achieve their goals.

What you’ll be doing

As a Health and Wellbeing Coordinator, you’ll work closely with individuals to understand their needs, goals and aspirations, developing personalised outcome plans that support positive and lasting change.

Acting as a keyworker and advocate, you’ll build positive, trusting relationships and support people to develop their independence, resilience and the skills they need to maintain their tenancy. You’ll promote health and wellbeing, social inclusion and informed choice, while connecting individuals with local services, community resources and opportunities.

You’ll also work collaboratively with external agencies to ensure people receive coordinated, holistic support, while helping us continue to deliver a high-quality service in line with professional and regulatory standards.

We’re looking for someone who is non-judgemental, resilient and solution-focused, with a genuine commitment to empowering people and putting the individual at the heart of their support. You’ll have experience of working with vulnerable people, which could include homelessness, tenancy support, mental health, addictions and learning disabilities.
