About the role

We currently have an exciting opportunity for talented Health and Wellbeing Co-ordinators, to join our team at the WAYfinder North East Outreach Service as we embark on the next stage of our journey delivering truly outstanding, highly personalised care and support services. WAYfinder North East Outreach Service supports up to 664 households living in homeless accommodation who have complex needs, such as mental health, addiction, with a focus on engagement and housing support.
We are looking for Health and Wellbeing Co-Ordinators to join our team that are motivated and committed to providing person centred support. As a Health and Wellbeing Co-ordinator, you will join a team of other co-ordinators to become the named keyworker for individuals accessing support and will utilise the outcome star assessment tool to co-create personalised support plans to encourage people we work for to achieve their goals.

Who are we looking for?
The ideal candidate will:
- have experience of working within health and social care and demonstrate an innovative and empathetic approach to delivering and leading services;
- have a passion and commitment to delivering person-centred support with a trauma informed approach;
- have experience of working as part of a team, or lone working and experience of co-production of support plans to enable individuals to meet and achieve their goals and aspirations;
- be able to assist in delivering an outstanding service; and
- have experience or knowledge of the effects homelessness and addiction can bring to individuals.
