About the Role

Wheatley Care is committed to delivering high-quality, person-centred support that enables people to live fulfilling and independent lives. We are currently looking for a health and wellbeing coordinator to join our Personalised Self-Directed Support Service – Mental Health.

The service is registered with the Care Inspectorate and provides housing support and care at home services to adults and older people with mental health needs, substance use issues, physical disabilities, and other vulnerabilities, supporting them to live independently in their own homes.

As a health and wellbeing coordinator, you will play a key role in supporting individuals to achieve their personal outcomes and aspirations. Working in line with the Health and Social Care Standards, you will adopt a trauma-informed, strengths-based and non-discriminatory approach, ensuring that support is co-designed and outcome-focused. You will act as a keyworker, developing personalised outcome plans and supporting individuals to build resilience, independence, and community connections.

What You’ll Do

  • Work in line with Health and Social Care Standards and organisational values;
  • Build trusted relationships and support individuals to achieve their personal outcomes;
  • Act as a keyworker and co-produce outcome plans, risk assessments and reviews;
  • Support individuals with daily living activities while promoting independence and choice;
  • Use outcome-focused tools (e.g. Outcome Star) to track progress and achievements;
  • Facilitate regular reviews and adapt support where required;
  • Promote community inclusion, access to services, volunteering and employment opportunities;
  • Work collaboratively with colleagues, families, carers, and external agencies;
  • Support multi-agency working and attend meetings as required;
  • Maintain accurate records and complete administrative tasks; and
  • Contribute to a safe, inclusive and supportive environment.

What We’re Looking For

Experience & Skills

  • Experience working with vulnerable individuals (e.g. mental health, addictions, homelessness, learning disabilities or social care);
  • Experience of team working and multi-agency collaboration;
  • Understanding of Self-Directed Support (Scotland) Act 2013;
  • Strong communication, relationship-building and negotiation skills;
  • Good time management and organisational skills;
  • Ability to work independently and as part of a team; and
  • IT skills (Microsoft Outlook, Word, Excel).

Qualifications

  • Commitment to achieving a relevant SSSC-recognised qualification, if not already held;
  • SVQ 2/3 Health & Social Care, HNC Social Care or equivalent (desirable); and
  • SSSC registration required upon commencing in the role.
