Wheatley Care's South Lanarkshire Mental Health service provides care at home and housing support to adults experiencing mental health difficulties and learning disabilities living within the South Lanarkshire locale.

The service operates from 2 staff bases in Blantyre and Lanark. Individuals who use the service have various support needs and receive support within their own home, where staff provide emotional and practical support to individuals to develop, regain and maintain independent living skills and to integrate into the wider community.

Packages of support vary from a few hours per week up to 24-hour support. The support provided is responsive to individual need and the individual is the leader in their own support.

Through person centred planning the people we work for have a support package which is designed to meet their needs and personal outcomes as per their individual Outcome Star, which is reviewed regularly to ensure needs are continued to be met.

Staff within the team work in collaboration with the Health and Social Care partnership and other external agencies to ensure that support is person centred.

Wheatley Care's South Lanarkshire Mental Health service accepts referrals from South Lanarkshire Social Work Department, voluntary agencies and family and friends.  You or someone on your behalf can make a referral to the service.
